A UR should be better than a 4 star. +3 con is only an upgrade from + 2 CON if you con is odd now.

Given that there is a +4 STR, -1 DEX UR belt, 4 seems appropriate to me, as STR is generally more desirable than CON.

If there must be a downside, then perhaps +4 CON, -1 reflex

On hit, living enemies suffer ongoing 10 damage at then end of each subsequent round

Ranged mainhand - +4 Sling of the Profligate - All
5, 6, 7, 8
-2/-2 when used without ammunition

Charm of the Veteran:
+1 to HP and +1 to AC, and an additional +1 to HP and +1 to AC for every different dungeon you have done this season.

Or, alternatively:

When you pass a skill test, gain your choice of +2 to damage, AC, or saves for the remainder of the room (1/room)

I like this one, though auto passing skill checks may not be advisable if there are plans to scale or otherwise increase skill check power.

The alternative suggestion is good to, but could Healing be one of the increases?

Cloak / Ring / Amulet of Displacement - 50% miss chance vs. targeted attacks (1/game, 2/game?)

Ring of Mending - Heal +2 HP at the end of each room

Ring / Amulet / Charm of Berzerk - Until end of room +3 melee damage, -2 AC and Will (2/game) - Barbarian, Fighters

Ring of Alertness - Immune to Surprise, +1 Party Initiative

Ring/Amulet/Charm of Illumination - Ignore penalties due to darkness, 5 retribution damage vs. undead

Ring of Preservation: +2 to saves and AC

Ring of Teleportation: You are immune to retribution damage (maybe also undead touch attacks?)

Ring/Charm/Crown of Accuracy: +2 to hit with ranged attacks

Ring of Levitation: You have Featherfall and Water Walking

Armor:
Demonhide Armor - +6 AC and -2 from Fire and Darkrift - Barbarian, Bard, Druid, Fighters, Ranger, Rogue

This is one of those mechanics that has the potential to greatly slow down combat. Faster to either ignore hit 1/game or soak X damage.

Agree at the high level, most of these recent posts from me on this thread are aimed at players with 1 UR.

I think this is a great token at Normal for someone with 1 UR.

Based on the change to cloak of shadowskin I don't think we want stackable hit-ignorerers.

My Bard loves scrolls, and was looking for more tokens that would buff scroll damage. He already is using sage speed.

Situational UR is shit for majority of players.

Completely valid as a 4* transmute but completely wrong as an UR token.

I am honestly struggling of an UR that I would actually want, I mean I am going to buy the shirt for the transmute in a few years but I can't honestly say that I would buy any UR unless its required for the cleric legendary transmute, I have the gold saved up for it. So what does jeff do, more slotless ? That seems to just invoke power creep arguments, some selective reprints, I mean sure there is a few things that could be reprinted. All my big wants for reprints or slots for Cleric and Fighter have basically been taken. I am getting a Plat Nugget and a shirt next year and whatever I need to transmute and I don't honestly thing I am getting anything else besides that unless the fighter relix can beat a +3 str +4 con next item. I mean if fighter turns out to be a weapon thats decent. I am just maxed on charms and Ioun stones and I really don't seem me swapping any of them out.

+3 damage for scrolls and +3 healing for scrolls

-or-

+3 damage.
