AC 40 means you won't really get hit excepting crits on NM runs. Just the redoubt set is enough for AC for you. AC 35 means you will occassionally be hit but don't forget you have a healer there for a reason :) Don't be afraid to be hit in melee and ranged attacks. At NM+, the biggest danger is with saving throws and just not doing enough damage in the time allotted.

Items to change:
Mithral Bracers - The +3 to AC won't make much of a difference and there are a ton of wrist items that are much more valuable to an effective build (unless you are killing dragons - then a Mithral set is nice.)
Dirtdazzy's Boots - you won't get hit much with an AC of 37 so DR isn't as important
Belt of Thorns - your AC prevents this from being effective very often.
Figurine: Your reflex save is already solid - the pixie is nice in cases where it isn't - better options are available.
Ioun Stone Amber Cube - not useful as your fort is high
Kilt of Dungeonbane - switch to Str - you'll lose 2 on AC (meaning AC 35) and reflex saves but your reflex saves are already good and +2 to hit and +2 damage is pretty big for your build.
Shaed is good for lower AC classes - it is good but saves are your bigger danger.
Ioun Stone Beryl Prism - your saves will be helped by suggestions below - you'll need that slot available.
Gauntlets of Ogre Power - Good for now but you can do better.
Lenses of Heimdall Sight - good for now but upgrade to instant analysis when you can (the problem is those are expensive for just a +1 to hit.)
Shirt of the underdark - Hp isn't your biggest challenge, Shirt of the Oaf or Blessed Str will help more.

Recommended items:
Orion's belt is really useful - especially with the number of good charms and ioun stones available. It is fairly recent and readily available for around $250. Until then, a belt of the brave is vastly under-rated - it is +2 to hit and +2 damage - by far, among the best rares ever printed.
Charm bracers - because of the transmute, these will be best in slot for virtually all classes. Short term, it gives you 2 more charm slots that, even with rares, are big.
Boots of the Four Winds are, ultimately, the BiS for all feet slots for fighters but is expensive ($1K+). Until then, Death Knight Boots give +3 Str and will serve you well. Boots of shocking step (rare) are a good filler until you get death knight boots.
Figurine of Power Tortoise is likely BiS for most fighters for 1 of their figurine slots. It is hard to find and expensive, though. Less expensive is the UR reaver (likely $50) and is automatic damage 1/run. Also worth considering is Parrot (rare) - most will saves are charm - it's like a poor man's Tortoise. Other expensive options would be panther or cat - both are great but expensive.
Lucky Cloak - +3 to saves - easy and add a lucky charm and you gain +5 hp from the set bonus. The rare +2 resistance is cheap and easy to find until then.
Gloves of Weapon Finesse - to hit is a challenge for you right now - this helps immediately. They are hard to find, though, and likely run $200+. Until then, Death Knight Gauntlets are about $100 and give +2 to hit and +2 damage.
Shirt of Blessed Strength - relic but relatively inexpensive (less than $100.) Shirt of the Oaf is good too and has additional value that it is part of a multi-year transmute where the pieces are still possible to get reasonably.

Charms from your extra charm slots to consider:
Lucky Charm - +2 to saves and helps you get a set bonus
Charm of Awakened Synergy - no surprise, better init, and extra hp. Should cost around $200-$250.
Charm of Enlightenment - +4 to will saves but hard to find and expensive
Charm of Glory - rings slots are at a premium now
Charm of the Fire Newt - straight +2 melee damage - easy to make or buy
Charm of Epic Insults (UC - taunt 2 monsters)
Charm of the Earth (UC) - +1 damage

Ioun Stone Replacements/Additions:
Silver Nugget - get it while it is reasonably priced TEs always seem to go up in price and tend to pay for themselves
Infernal Fire +2 dmg
Jasper Ellipsoid (Rare) +2 dmg


your open 2 bead slots:
Cleric's Piety - hard to find but +4 to will saves is amazing
Guided Strike - fighters and rogues lucked out in getting this as a bead rather than wrist slot. When you get that nat 20 - X3 dmg as a dwarf is amazing - best not to waste it.

I recommend get your saves to around +18 for NM difficulty. You'll make 75% of your saves then. The hardest one will be will - the parrot is a great work around there.
Try to get your base attack to +15 minimum - better at +18.

Not sure Belt of the Brave is vastly underrated; just underpriced at the moment :)
