In the E3 descriptions for room 5 and room 7:

Those who die in the lower planes are "lost", their souls can not return to their original plane.

This results in a +1 AC for the monsters as the lost souls attach themselves to the nearest powerful being.

Does that mean they can't be revived? Potions and Mushrooms don't work?

I would have to re-read the module, but I believe that effect only happens when they die and are not revived. I believe revival items would work.
