If I do get nightmare ticket even though I'm not equipped for it what options are there?

There are no dedicated Nightmare-level runs anymore, all runs are Normal difficulty unless the party unanimously decides to play on a different difficulty level. We tried dedicated Nightmare-level runs for a few years, but that didn't work out.

I suppose it's hypothetically possible (but highly improbable) that if you were on the wait list you might get placed into a group that wants to run Nightmare. If you agree (and thereby make the vote unanimous), I would simply be honest with the party. Say, "I'm willing to run Nightmare with y'all, but I'm not equipped for that. Does anyone have some equipment I can borrow that will help us all succeed?"
Have you looked it up in the TDb ?

Personally I like the idea of an epic difficulty. I wouldnt give it any extra xp or token reward though for the reasons Raven mentioned. A unique button would probably be about as far as Id go.
