In 2013 our plans call for us to add an additional adventure to the event. There will be two consecutive adventure modules, and each module will have two variations of either a combat or puzzle orientation. The first module called “Lycans Afoot” tasks the party to travel through a dark forest in search of a tower, while the second module called “Golembane” challenges the party to reach the top of tower.

Sadly, unless we take an unexpected trip to the Negative Energy plane, I think you've lost your chance. No Smoak planned this year, or any further year, as far as I'm aware.

Of course, I'm sure we'll be seeing that awesome new dragon head again eventually, in some context.

I hit Smoak GOOD

DM - One more strike like that and she'll be down!

Smoak kills all but 1 of our group (1 HP left)

That player uses their action to pour the Elixir of Life they just so happen to find clutched in my Corpse's hand down my fried gullet.

I step up to Slide...

And the bell rings.

You can't fix stupid but you can TPK it.

Personally, Landspeeder_95, if I had been your DM there I would have said, "You have exactly one second to slide."

I DMed the final "option" room for the Retro run at True Realm (I met the party in a buffer room and told them to choose a final puzzle or a final combat, pick a door...major props to my NPCs for that, because they had to be listening through a curtain to know if they needed to crawl under a tiny table or run to the next room and pull on a bulky costume) and ended up dropping quite a few characters and a couple entire parties there (especially with the puzzle; it made sense but wasn't exactly the easiest thing to figure out under stress), but I've never ran a final room at GenCon.

I have killed my share of PCs, though. Mostly while running combat for the Medusa, the Illithid, and the Sirens. I got a TPK on a Nightmare group with the Illithid (I feel no shame in wiping a Nightmare group because the whole point of Nightmare is that they're asking for the challenge, and at least a few of that group made sure to express their begrudging respect for the difficulty level). I presided over a TPK in the Medusa room, though that TPK was all Helene's (NPC) doing...she got the entire party save one turned to stone (IIRC she got two of them the second time around) and the last one just wasn't a match for the combat part while going solo.
Everybody gets to make fortitude saves.  Again.  Also, mind your step to avoid the pixie, lest you risk eldritch immolation.
