For the Hat of Escape, what exactly qualifies as an "attack"? Melee and Ranged attacks seem a given, but what about spells? How about other special abilities? (I can't think of anything specific right now, but I'm covering the bases.)

Anything that would require the focused attention of the monster, specifically a die roll.  Inherant attacks like appearance, or some sort of natural effect like scent or sense of fear (things you would likely have to save against without a die roll to hit on the part of the monster) would not qualify.
You should know better than to pick up a duck in a dungeon....

So a spell such as Magic Missile that doesn't require a roll (that I know of) would not be protected against, but a spell such as Scorching Ray would?

Specifically cast spells (e.g., magic missile) would still be affected.  Again, anything that would require the focused attention of the monster.  I should have said specifically a die roll or cast spell.
You should know better than to pick up a duck in a dungeon....
