My dnd knowledge is lacking I admit. My search-fu is solid but I couldn't find anything. Anyone have an idea what a Lost One is?

If I were to guess it would be a play on some type of lost soul creature. The D&D wiki describes them as follows

Lost souls are tortured victims of Hell that have been weaponized by demons. They resemble floating, flaming skulls. They float aimlessly in silent anguish until they see a living creature. They can attack from some distance away if they move and then perform a screaming charge. They are usually encountered in groups.

Given where were going it seems likely it could be an effect that prevents player revival

We’ve seen at least two so far. Well, the same one twice.

One was on the way to the beholder. He begged us to destroy him before he lost his will.

We saw him again in the Astral plane (N1)

I believe that monster was called a Fallen One, not a Lost One. It was a former angel that the mind slayer captured and enslaved, and then their soul was freed to the Astral Plane after the players killed it. Perhaps during N1 it changed to a Lost One.

That sounds more on spot to me and I have a module and looked and yes they were Fallen Ones. Thank you Hawk for the reminder.
