well, i narrowed down my guesses to three options, so lets make those my three guesses :)

1000GP sapphire necklace
celestial bracers
celestial girdle

Mithral Gauntlets? They'd be from 2010, from what I can tell.

My other guesses are Boots of the Underdark and Celestial Bracers (not sure if Celestial counts as a creature type).

my guess is celestial girdle or scepter of the dead

Edit: now that I see you are counting the past 3 years as 2013,2012,2011 and 2010 celestial girdle is out, but I still am going with scepter of the dead.

I read the last three years as 2013, 2012, 2011. So 2009 should have been valid. Are we dealing with a miscount? Or trying to be sneaky and allow 2013s? Couldn't get it the past 3 years but could get it this year.

Won't be Medallion of Greyhawk because it can't be a neck slot item that prevents you from wearing the horn of plenty which you need for full treasure pulls.

I read past three years as 2013/2012, 2012/2011, 2011/2010. But apparently was mistaken or it could mean it is a token from this year. Also, it doesn't have to be an UR. It could be a rare (a valuable one), completion token, exalted, etc
