I proposed something called "Nightmare-plus" several months ago.
Works like this:
Make up a little 3x5 card that says "nightmare plus X" and choose any number for X.
On the card, also write "AC minus X, To Hit minus X, Damage minus X, All Saves Minus X" and hand it to the coach filling out the party card. And have all your characters preprinted. Don't give the coaches the extra work if you haven't made it as easy as possible on them. Everyone in the party gets the penalties you chose.
As far as the the DMs are concerned, they play it like a regular nightmare run. So do you, except when they tell you to take damage, take an extra X.
There would be absolutely no extra rewards, treasure nor buttons, for playing this way. Just bragging rights. "We beat it on nightmare plus 20 with only eight fatalities!"
Pretty simple, really. You could apply the same idea to any level, I suppose, and only be as bored as you want to be.
