TOPIC: Transmuted Tokens Questions

Does anyone know where the form is that we should fill out for transmuting tokens onsite at WYC? I see the online ordering form link for submitting them via mail, but I don't see the form to fill out for onsite transmuting.

Also, are we still able to transmute the Druid's Wild Vestments for the Shirt of the Fae? I recall it was said that we could, but I don't see it on the Transmuted Tokens page (and it does have the Gloves of Dexterity for the Gloves of the Cutpurse transmute, which is similar).

I don't think the form for tdc has been released yet.

I also don't remember that about the shirts, yhough you could search the the forums.
