1) On the Bone Scroll Case and Alchemists Pouch protecting Scrolls and Potions, has there ever been a situation in a True Dungeon adventure where these have been used?

I've never seen the Alchemist's Pouch used. I've only ever used a scroll case for the Dome and Key puzzle. Coincidentally, the Dome and Key puzzle was also the only opportunity I've ever encountered to use a Mortar and Pestle in a dungeon. (You got an actual M&P, which could be used as a weight with the rope or twine.)

2) On the Boots of the Leopard giving half-damage from Mechanical Traps, has there ever been a situation in a True Dungeon adventure where these have been used? Seems like most traps were magic or poison (or push damage, which I presume you can't resist?)

This brings up a good point. The Boots of the Leopard sound pretty cool, but has there EVER been a mechanical trap that was clearly designated as such? Are the Rogue traps ever mechanical? (Still, probably not worth the trade-off with Boots of Elvenkind/Underdark.) I'd really like to see some cool mechanical traps in a dungeon.

3) Potion of Water Breathing, was there ever an underwater encounter where this was used? I would also presume the Ioun Stone of Iridescent Spindle would work for water breathing as well?

Also, by jumping in the water with PoWB or Irridescent Spindle, it was possible for a character to avoid the Stirge combat completely. Though, not everyone would fit in the water.
I think the Iridescent Spindle also works against Stinking Cloud (and other poison gas attacks).

6) The ring of feather falling, has this ever been used in an adventure?

As I recall, the Mind Flayer pit was not bottomless and Feather Fall worked well there. The windy chasm in '04 may have been very deep or bottomless, but I think Feather Fall could slow your descent long enough that the party could toss you a rope. So, the FF could save you, even if the pit was very deep (or even bottomless).
The Ring of Feather Fall is a nice ring to have with you in case you encounter a pit or chasm. It's not one I'd normally wear as part of a general setup, though.
