As of right now, Questor's Charm of Luck no longer lets you re-re-roll 1's. If you get a 1 on the re-roll, you fail the save. The text printed on the token clearly says a 1 can be re-rolled once. This is the exact wording:
+1 to saves & re-roll any save once if "1" is rolled

We realize this will be a disappointment for some folks. However, we think it's best for there to be some sense of danger, even for über-powerful characters.

This is the way the token was designed to be played. I take responsibility for the misinterpretation. I sincerely apologize for my error and for not catching the error sooner.

I'm confused.
Does this mean Questor's only provides a +1 to saves as its sole function or was your clarification that a 1 can only be re-rolled once?

I'm assuming the 2nd one.

Anyone with a Questor's now has a 1 in 400 chance of actually rolling a one on a save (barring Grace, Widseth's Legendary, etc)

The latter.
Questor's Charm of Luck still lets you re-roll a natural 1, but you can no longer re-re-roll natural 1's. To put it another way, even if you have a +100 to your saving throws, when you re-roll a save with Questor's, if the re-roll result is a natural 1, you fail the save and cannot re-roll the crappy re-roll.

Before this correction, if one had Questor's and a sufficiently high saving throw bonus, one could automatically succeed on any saving throw. Sure, one could always fail on a 1, but since Questor's (mistakenly) gave you infinite re-rolls of 1's, you didn't have to bother rolling a save because you would eventually roll a not 1.

Just for clarity another token (like Wade pointed out) could let you reroll a rerolled 1 correct?

This is the way I always interpreted it. Was there a ruling somewhere that I missed that said "You get infinite re-rolls" or did that just happen once or twice in a dungeon by accident?

There was (and no longer is) a clarification on TokenDB that said it applied to consecutive 1s. I don't know when it showed up, but I recall thinking it strange when I saw it earlier this year, since I didn't think it worked that way.

Currently, because there isn't a rule* specifically stating otherwise, that's the case. But I wouldn't bank on it staying that way. One mulligan is fine. Using loopholes to re-roll ad infinitum is a sleaze that needs to be eliminated.

*Some tokens specifically say they have limited uses. E.g., Anti-toxin clearly says it's only usable 1/room. As always, a specific token rule overrides a general rule.

I like that. There should be some element of danger. I wouldn't mind even more danger being added - like maybe a monster always hits on a 18-20, regardless of someone's AC.

I disagree just because everyone likes to play their own way. If you want to wear legendary armor on normal difficulty so you cannot be hit (except by a crit hit), that is your choice. You spent the money and/or resources on the token and the ticket for the adventure.

That said personally I like to play with my group on a level that is a challenge for us, but not everyone does and I wouldn't want to push the way I like to play on anyone else as much as I wouldn't want anyone to push their style on us.

I'm ok with the change. For those wondering, the infinite rolls were specified in tokendb for as long as I can remember.
