I did a quick search of the "ingredient for" associations, and I found these discrepancies. These assume that the description from TDb should be reflected. I may have missed some, but I believe these are accurate:
3-baton-of-focus is missing philosophers-stone, wand-of-lava-blasts
3-mithral-bracers is missing 2-mithral-bracers, 2-celestial-bracers
belt-of-baneful-mettle is missing mystic-silk and has extra elder-orb-eye, lava-beast-nugget, philosophers-stone, spidaur-venum, succubus-horn
belt-of-blessed-constitution is missing alchemists-parchment
draco-lich-claw-charm is missing darkrift-runestone, frost-worm-tooth, spectre-essence, troll-tusk
gem-of-blinding has extra 30-gp-coral
girdle-of-frost-giant-strength is missing armor-of-the-lynx, girdle-of-dwarven-health, mystic-silk
greater-cloak-of-destiny is missing elven-bismuth
khings-ring-of-supreme-evasion is missing elven-bismuth, enchanters-munition
Also, based on descriptions like the one for Potion Distilled Healing, I'm assuming you'd like to expand recipe items like "any ultra rare shield" to the actual associations - as long as the yearly upkeep isn't too much of a pain. If so, I'd recommend expanding the recipes for the following:
3-rod-of-niltongue (any ultra rare melee weapon)
blessed-redoubt-shield (any ultra rare shield)
dagger-of-throwing (any bladed weapon)
greater-alchemist-pouch (any non-scroll-healing)
ring-of-heroism (any rare ring or GP "ring")
starhide-robe (any ultra rare robe)
As I said, I likely haven't got everything 100% right; this was a bit unwieldy to cross-check. I may look into creating a series of associations that model "recipes" - including item amount, gold amount, last day to transmute, and the ingredient choices ("any of" / "6 different completion tokens" / etc)
